優秀な研修医マーティン・ブレイク(オーランド・ブルーム)は、肝臓の感染症で入院してきた18歳の少女ダイアン(ライリー・キーオ)の担当をまかされる。マーティンは自分に絶対の尊敬と信頼を示してくれるダイアンと接するうちに、特別な思いを抱くようになる。その後マーティンの献身的な治療によってダイアンは順調に回復し、退院するのだが、その頃にはマーティンの医師としての熱意は狂気に代わっていた。マーティンはダイアンを取り戻すため、薬をすり替えて病気を再発させ、再入院した病院の点滴にも細工を施したのだ。「退院させない――。」マーティンの野望はやがて取り返しのつかない事態を引き起こし、警察が動き出す。しかし、周りからは熱心に看護する医者に見えていたマーティンは疑われること無くキャリアを積んでゆく。そんな中、掃除係のジミー(マイケル・ベーニャ)がダイアンの日記を発見し、マーティンを脅迫しだす。マーティンは真実を永遠に闇に葬るため、再び禁断の領域で神の手を振りかざす…。果たしてマーティンの完全犯罪は成功するのだろうか？
